加纳报道首例高致病性H9N2禽流感

一月初, 加纳报道了一例高致病性H9N2禽流感, 四千多只蛋鸡染病死亡,扑杀一万多只。加纳地处西非,热带气候,爆发地点位于西部高原。非洲流行的多为G1谱系的H9N2病毒, 见下图。
H9N2禽流感可感染多种家禽,包括鸭子、鸡、火鸡、鹌鹑、鸽子、山鸡和鹧鸪等,并且在亚、欧、非和美洲都广泛流传 (Homme et al., 1970; Guan et al., 1999; Nili et al., 2003; Kim et al., 2006; Aamir et al., 2007)。一般常见的高致病性禽流感病毒为H5或H7亚型。自1966年首次监测到H9N2禽流感 (Homme et al., 1970),加纳的案例为首个被发现的高致病性H9N2病毒。
低致病性H9N2感染鸡可以引起呼吸道疾病,导致蛋鸡产蛋量下降,甚至死亡 (Nili et al., 2003; Kim et al., 2006; Aamir et al., 2007; Bianca et al., 2017),即使在使用H9疫苗进行防治的情况下,受病毒感染的鸡仍会生病或影响产蛋率 (Pu et al., 2015)。一旦H9N2病毒突变成为高致病性病毒,对禽养殖业的影响将更大,病毒感染可以直接导致禽类死亡,导致更严重的直接经济损失。由于高/低致病性是针对鸡的判断标准,高致病性H9N2若感染人会不会导致病情比低致病性病毒更严重,需要进一步研究方可判定。
1994年前后,H9N2病毒就已经开始在中国广泛流传,并且已经带有哺乳类受体突变 (Matrosovich et al., 2001),可以结合人的细胞表面受体,感染哺乳类细胞。自1999年以来 (Peiris et al., 1999),H9N2感染人的病例时有报道,只是一直未有发生人传染人的病例,并且病情都较轻,病人短期就痊愈。根据血清学监测,有相当一部分的人都可能感染过H9N2禽流感,只是由于症状不明显而被忽视了 (Liang et al., 2002; Chen et al., 2008; Gomaa et al., 2014)。鉴于H9N2在多个物种间的广泛流传及其能够结合哺乳类细胞受体的能力,H9N2病毒已经被列入预防流感大流行的候选亚型之一,根据病毒的流行态势,世界卫生组织亦会定期评核可用于人的H9N2疫苗候选株。
关于H9N2病毒在全球动物和人群中的分布可以参考链接的 这张图 (Khan et al., 2015)。此图虽然是1997到2013年的统计数据,但基本可以看出亚、欧、非和美洲都曾在禽类中检测到H9N2病毒,且各大洲都有人感染H9N2病毒的血清学数据。在中国,除了血清学数据,H9N2病毒都曾在人、猪、狗、野鸟等动物身上检测到。
除了OIE的初步通告,关于此高致病性H9N2禽流感的基因序列和相关科研文章尚未有进一步报道。待查。
